Overview

Manager of Care Management (RN or MSW) at our medical center in Fullerton, CA is a full-time position that will lead and manage the Care Management Team of RNs and Social Workers.

Specific Skills
Required qualifications:
  • Bachelor’s degree in nursing or master's degree in social work.
  • 3 years of Experience in an ambulatory or acute care setting.
  • Strong project management and operations experience.
Preferred qualifications:
  • Master's degree in nursing or health administration.
  • Case Management Certification (CCM or ACM) upon hire
  • 3 years of Management experience in a Healthcare setting.
Responsible For
  • Oversee and manage daily operations of the Care Management team, ensuring adherence to best practices and regulatory standards.
  • Provide leadership, guidance, and support to Case Managers and Care Management Coordinators.
  • Ensure effective case management processes to advocate for patients, facilitate access to appropriate care, and optimize resource utilization.
  • Collaborate with interdisciplinary teams to enhance care coordination and improve patient outcomes.
  • Monitor performance metrics and implement process improvements to drive efficiency and quality of care.
  • Serve as a key resource for resolving complex patient care and discharge planning issues.
  • Maintain compliance with all federal, state, and local regulations related to care management.
Additional Requirements
  • Holds a California RN license or California Social Work license?
  • Do you have minimum 3 years experience of working as a RN or as a Social Worker in a hospital/in-patient setting?
  • Do you have minimum 3 years experience working as a RN or Social Worker directly in Case Management or Discharge Planning in a hospital/inpatient setting?
  • Do you have minimum 1 year experience in leadership and managing people directly? This is a large part of the Manager of Care Management position.
